ABSTRACT

Objective To investigate the cognition, attitudes and clinical practice of ICU medical staff on the family meeting for ICU patients. Methods The study used questionnaire to investigate 428 ICU medical staff and SPSS software to analyze data. Results The cognition of medical staff on the family meeting was 25.4%(109/428). The attitudes of medical staff on the family meeting were positive. Only 38.17%(164/428)medical staff selected necessary or very necessary to carry out the family meeting and only 41.45%(177/428)medical staff would to participate in family meeting. Different professional, the highest record of formal schooling and ICU working life had different attitude scores(t=7.33,F=4.06,5.33, P<0.01 or 0.05);different professional titles and ICU working life had different aspiration attitude scores (F=4.79,6.94,P<0.05 or 0.01). The rate of family meeting in clinical work was only 8.18% (35/428). Conclusion Medical staff has low level of cognition and attitudes about family meeting on ICU patients, and still many difficulties in clinical implementation.

ABSTRACT

Objective To evaluate the effect of individualized rehabilitation extended nursing on activities of daily living (ADL) in children with cerebral palsy. Methods A total of 156 cases of non-operative children with severe disorder of cerebral palsy from January to December 2017 in Sichuan 81 Rehabilitation Center, of 101 males and 55 females, aged 2-7 years with weighting 9 to 21 kg, were divided into 2 groups according to the time sequence, of which 70 cases of January to June 2017 before extension nursing as control group and 86 cases of July to December 2017 after extension nursing as observation group. The control group was received routine nursing according to the ADL rating scale, and the observation group was adopted the extended nursing method, according to the ADL rating scale to evaluate and judge the barrier grade, and chosen the suitable measures for rehabilitation nursing according to the individualized principle. The rehabilitation assessment was carried out before extended nursing (W0), the second week after extended nursing (W2), fourth week (W4), sixth week (W6), eighth week (W8), tenth week (W10), and twelfth week (W12) respectively. Results There was no significant difference in gender, age, weight and ADL scores at the time point of W0between the two groups (P>0.05). The ADL scores of the two groups were increased with the treatment time, and the increase of the observation group was greater than that of the control group. Compared with the control group, the ADL scores of the observation group were increased significantly at the time of W4to W12(P<0.01).After 12 weeks of treatment, there were 25 cases of mild disorders (29% ), 46 cases of moderate disorder (53.5% ) and 15 cases of severe disorder (17.5% ) in the observation group, and 3 cases of mild disorders (4.3% ), 38 cases of moderate disorder (54.3% ) and 29 cases of severe disorder (41.4% ) in the control group, and the observation group was obviously better than that of the control group (χ2=21.083,P<0.01). Conclusion Individualized rehabilitation extended nursing can improve ADL in children with cerebral palsy.

ABSTRACT

This study was aimed to improve the transfer rate of sinomenine in the intermediate product of Caulis Sinomenii in order to optimize the purification process of intermediate product ofCaulis Sinomenii fromTong-An (TA) injection. The transfer rate of sinomenine and the stability of fingerprints in the intermediate product of Caulis Sinomenii were used as indexes for the investigation on the impact from different pH ofCaulis Sinomenii extract before extraction. The transfer rate of sinomenine was used as an index for the investigation on the impact by different pH of hydrochloric acid created to dry extract solution. The transfer rate of sinomenine was used as an index for the investigation of four separation ways, which included the vacuum filtration, plate and frame filters, high-speed tube separator, and flat direct centrifuge, on the liquid separation of sinomenium acutum acid. The results showed that the pH ofCaulis Sinomenii extract before extraction was 10-11; the transfer rate of sinomenine was the highest in the extraction process and the fingerprints of TA injection was stable. The pH of hydrochloric acid was 2.0-2.5; and the highest transfer rate of sinomenine in acid dissolution process was 92.94%. The high-speed tube separator had the best separation to sinomenium acutum acid-dissolving liquid. The highest transfer rate of sinomenine was 93.34%. It was concluded that the optimized process can effectively improve the transfer rate of sinomenine in the intermediate product ofCaulis Sinomenii. Meanwhile, fingerprints of the product were stable. The process was simple with good repeatability.

ABSTRACT

Objective To explore the clinical value of 3-dimensional digital subtraction angiography(3D-DSA)in the diagnosis and therapy of intracranial aneurysms. Methods 2-dimensional digital subtraction angiography (2D-DSA)and 3D-DSA was performed respectively on 1 53 patients, including 1 5 1 cases with spontaneous subarachnoid hemorrhage (SAH),1 case with headache and 1 case with cerebral infarction;Meanwhile,to compare the difference in diagnosis of intracranial aneurysms between 2D-DSA and 3D-DSA.Results 1 12 patients(125 pieces)with intracranial aneurysms were detected on 2D-DSA,its detecting rate was 73.2%(1 12/1 53 );136 patients(1 52 pieces)with intracranial aneurysms were detected on 3D-DSA,its detecting rate was 88.9%(136/1 53)(χ2 =12.254,P <0.05).The displaying rate of 3D-DSA for the relationship between carrying tumor artery and aneurysms and the necks of aneurysms was 100%(1 52/1 52),however,the displaying rate of 2D-DSA for that was 61.6%(77/125)(χ2 =63.993,P <0.05). Conclusion 3D-DSA can improved the diagnostic accurate rate of intracranial aneurysms,it is important in chosing the therapeutic regimen.

ABSTRACT

AIM To study the therapeutic effect of bone marrow transplantation on mustard gas-poisoned mice. METHODS Mustard-poisoned mice model was constructed by injected mustard gas 20 mg·kg~(-1) subcutaneously. At 4 h, 1 d and 2 d after poisoned by mustard gas, the suspended bone marrow cells (106 cells per time) were injected into mice respectively. After poisoned 3, 5, 7 and 10 d, the numbers of leukocyte and marrow nucleated cells were recorded periodically. At the same time, after poisoned 4 h and 1 d by mustard gas, mice in another group were injected granulocyte colony stimulating factor (G-CSF) 74 μg·kg~(-1) combined with transplantation and changes of the cell number were observed. RESULTS Compared with poisoned group without transplantion, leukocyte number in bone marrow transplanted group presented ascending trend, but there was no statistical significance until the 10 d after poisoned. However, the number of marrow nucleated cell increased significantly from the 5th day. After mice injected bone marrow cells combined with G-CSF, leukocyte increased markedly in earlier stage compared with transplantation only. CONCLUTION The bone marrow transplantation has significant effects on improvement of marrow hematogenic system inhibited by mustard gas.
